I understand that participation in Next Level Lax Camp involves physical activity and inherent risks, including but not limited to falls, contact with other participants, use of lacrosse equipment, and potential injury.

I certify that my child is physically fit and able to participate. I agree to inform camp staff of any medical conditions, allergies, or limitations that may affect my child's safe participation.

I authorize camp staff to seek emergency medical treatment for my child if necessary and agree to assume responsibility for any related medical expenses.

I release and hold harmless Next Level Lax Camp, its director Fisher Finley, coaches, staff, volunteers, and Milton High School from any claims or liability arising from participation, to the fullest extent permitted by law.

I grant permission for my child's image or likeness to be used in photographs or video taken during camp for promotional purposes.
